    Course Description:

    This training educate learners on the philosophy and practical application of person-centered care by using experiential case studies to effectively write person centered assessments and develop plan of care (also referred to as treatment plans).

    Course Objective:

    At the end of this training, learners will be able:

    • Describe the benefits of a person-centered approach to treatment planning.
    • Differentiate between person-centered treatment and traditional approaches.
    • Participants will complete and interpretive summary by addressing of it it’s components.
    • Write effective person-centered, SMART objectives.

    Intended Audience:

    Direct Service providers involved in assessing and developing a plan of care; quality assurance staff, clinical supervisors
